The 37th EAU Congress was successfully held from July 1 to July 4, 2022 in Amsterdam, Netherlands. 

Founded in 1973, the European Association of Urology (EAU) is the largest and most inϐluential urological meeting in Europe. EAU is dedicated to improving the treatment of urological diseases in Europe, its urological guidelines are widely used by urological institutions and practitioners in Europe and even around the world.

Prostate biopsy-related infection rate

The diagnostic section of the guideline states that seven randomized studies enrolling a total of 1,330 patients compared the biopsy route on infectious complications. Infectious complications were signiϐicantly higher (RR: 1.81 [range 1.09-3.00], 95% CI) for transrectal biopsies (37 of 657 men) compared with transperineal biopsies (22 of 673 men) [311-318]. In addition, a systematic evaluation that included 165 studies containing 162,577 patients found sepsis rates of 0.1% and 0.9% for transperineal and transrectal biopsies, respectively [319]. Finally, a population-based study in the United Kingdom showed that 486,467 biopsies were performed over a 10-year period from 2008-2019, and showed lower rates of sepsis and infection for transperineal versus transrectal biopsies (0.53% vs. 0.31%, p < 0.001) [320]. The available evidence suggests that the transrectal approach should be abandoned in favor of the transperineal approach despite any possible logistical challenges.

The intensity levels of the above recommendations were determined by the primary clinical signiϐicance of them. Although the transperineal puncture is a low recommendation based on the data showing a lower infection risk, however, regarding the statistical and clinical signiϐicance, a strong recommendation is given to the transperineal puncture. Although data quality is low, routine surgical skin disinfection by transperineal puncture and rectal cleansing with povidoneiodine by transrectal biopsy are also given as strong recommendations because of the high clinical beneϐit and procedure simplicity. Avoidance of ϐluoroquinolones in prostate puncture is given as a strong recommendation due to EU law.
